PAHO Alert: Simultaneous Flu & RSV Surge in the Americas - What You Need to Know (2026)

Imagine a winter where two respiratory viruses team up to challenge our health systems. That's the reality the Americas are facing right now. The Pan American Health Organization (PAHO) has issued a stark warning about the simultaneous spread of seasonal influenza and respiratory syncytial virus (RSV), a situation that could overwhelm hospitals and clinics across the region for the rest of the winter.

This alert, released on January 10, 2026, builds upon a previous advisory from December 4, 2025, which foreshadowed a potentially earlier and more intense respiratory season than usual. You can find the full details in their epidemiological alert: [https://www.paho.org/en/documents/epidemiological-alert-simultaneous-circulation-seasonal-influenza-and-respiratory].

Globally, influenza has been on the rise since October 2025, with the A(H3N2) strain leading the charge. Several Northern Hemisphere countries are already seeing early signs of seasonal activity, and RSV cases are creeping upwards too.

Here’s where it gets concerning: In the Americas, influenza positivity rates remain above 10% in the Northern Hemisphere, with North and Central America experiencing sustained increases. The Caribbean is particularly hard-hit, with rates nearing 20%, dominated by the A(H3N2) strain.

Data from key countries like the United States, Canada, the United Kingdom, and Spain reveal an unusually early and aggressive start to the flu season. Outpatient visits, especially among children, are surging, and hospitalizations—particularly among older adults—are on the rise.

While these trends align with typical seasonal patterns and overall severity remains similar to past years, some countries are reporting activity levels—measured by flu detection and outpatient visits for flu-like symptoms—that surpass recent seasons.

And this is the part most people miss: The gradual uptick in RSV cases could further strain already stretched health systems. This makes continuous monitoring absolutely critical to adjust healthcare responses effectively.

“The dual circulation of influenza and RSV is a major challenge,” explains Dr. Marc Rondy, PAHO’s Regional Adviser in Epidemiology of Epidemic- and Pandemic-Prone Diseases. “We must prioritize vaccination to prevent severe cases that could lead to hospitalization, and maintain vigilant surveillance to act swiftly and prevent larger outbreaks.”

PAHO stresses that interim studies confirm current flu vaccines are effective in preventing hospitalizations—30–40% in adults and a remarkable 75% in children. They urge countries to ramp up vaccination coverage, especially for vulnerable groups like children, pregnant individuals, older adults, and those with chronic conditions.

Given this complex situation, PAHO recommends that countries in the region take the following steps:

  1. Strengthen integrated surveillance of influenza, RSV, SARS-CoV-2, and other respiratory viruses, reporting weekly data to FluNET and FluID for regional and global monitoring.
  2. Prepare and adjust healthcare response plans to handle potential simultaneous surges in influenza and RSV cases and hospitalizations.
  3. Prioritize influenza and COVID-19 vaccination for at-risk groups, including older adults, young children, pregnant individuals, people with chronic conditions, and healthcare workers.
  4. Implement RSV prevention strategies, such as maternal vaccination and long-acting monoclonal antibodies for newborns and infants, following PAHO/WHO guidelines.
  5. Enhance risk communication by promoting essential preventive measures.

PAHO reminds everyone that simple actions like getting vaccinated against influenza, frequent handwashing, covering coughs and sneezes, wearing masks indoors if symptomatic, staying home when sick, and seeking prompt medical care for severe symptoms can effectively protect you and your loved ones, especially young children and older adults.

But here's where it gets controversial: With healthcare systems already under pressure, should countries consider mandatory vaccination for certain groups? Or is this a step too far? And how do we balance individual freedoms with the collective need to protect public health?

What do you think? Are we doing enough to prepare for this dual threat? Share your thoughts in the comments—let’s spark a conversation that could shape how we tackle respiratory viruses in the future.
